HTCI SCHOLARSHIP PROGRAM FOR YOUTH VOLUNTEERS

VOLUNTEER WAIVER AND RELEASE OF LIABILITY

1. Assumption of Risk

I, the undersigned participant (or parent/guardian, if the participant is under 18), understand and acknowledge that participation in volunteer activities at HTCI involves certain risks, including but not limited to physical injury, illness, or accidents that may occur while performing volunteer duties. I voluntarily assume full responsibility for any risks associated with participation in this program.

I hereby acknowledge this risk and waive any claims against HTCI for any allergic reaction my child may experience due to potential exposure to allergens. 

At the time of signing this form my child is a high school junior or senior.

2. Release and Waiver of Liability

In consideration of being allowed to participate in the HTCI Scholarship Program for Youth Volunteers, I, on behalf of myself (and/or my minor child), my heirs, assigns, and personal representatives, hereby release, waive, and forever discharge HTCI, its directors, officers, employees, volunteers, sponsors, and affiliates from any and all liability, claims, demands, actions, or rights of action that arise out of or relate to any injury, illness, loss, damage, or harm incurred during or as a result of my participation in the program.

3. Medical Treatment Authorization

I understand that in the event of a medical emergency, HTCI representatives may contact emergency services on my (or my child’s) behalf. I give permission for medical treatment to be administered if necessary. I understand that I am responsible for any medical expenses incurred and that HTCI is not liable for any medical treatment or costs.

4. Code of Conduct Agreement

I agree to abide by the rules, regulations, and expectations set forth by HTCI. I understand that failure to follow these guidelines, including the scholarship disqualification criteria, may result in termination of my participation in the program. I acknowledge that HTCI reserves the right to make final determinations regarding any violations.

5. Parental/Guardian Consent (for Volunteers Under 18 Years Old)

As the parent or legal guardian of the above-named minor participant, I acknowledge and agree to the terms of this waiver on behalf of my child. I understand and accept the risks involved in my child’s participation and release HTCI from any liability.
